I look into other people’s windows. I’d like to be delicate about admitting this, but there isn’t a delicate way, so: I look. On the evening walk, at the hour when the light goes and the lamps come on but nobody’s thought to draw the curtains yet. From the car, at a red light, at the second-floor apartment with the good bookshelves. A peeping Tom wants the bedroom. I want the living room — the lamp, the wall color, the painting with the striking frame, whether the television is the altar of the room or hidden away in shame.

What I take isn’t something you could report stolen. A dog, gray at the muzzle, keeping the night’s watch over an empty street. A woman I’ll never meet, standing at her own counter, doing the thing I was about to go home and do. It lasts the length of a footstep or five. Then I’ve passed, and I’ve built her a whole life on the evidence of one painting and a fern, and I’ll never be corrected, which is the wonder and pleasure of it.

It travels with me. In Hanoi the houses go up instead of out — narrow as a wardrobe, five separate lives stacked one above another, and you have to crane to read them. I craned. In a certain kind of Indian apartment block, the whole face of the building burns the same blue-white “tube light”, the light of the waiting room and the exam hall, dozens of identical squares glowing the identical institutional white.

And then, on the fourth floor, one window gold.

Somebody up there had decided against the light everyone else accepted. Bought a lamp. Chose warm over efficient, in a country where the tube light is simply what a ceiling has. I stood in the street for a long time, looking at a stranger’s lampshade, and understood for the first time what I’d been doing on all those walks. I wasn’t imagining lives. I was looking for that window — the one that defied the blue and got gold. In every grid I’ve ever passed: small, stubborn proofs that a person, handed the norm, went and found something better.

Here’s the part I didn’t see coming, standing in that street. You can’t do this to your own house. You’re out on the pavement in the dark, doing the looking, and your own window is behind you — you have no idea, from the inside, which light you’re throwing. Blue or gold. Whether the woman walking past yours slows down.

So I keep walking. Being let in was never the appeal — the fern was doing fine without me, and I had my own counter to get back to. I walk to keep count. There are more gold lamps than the tube lights would have you believe, and I’ve decided that this count is worth keeping.

Pour something. Let the curtains remain open.

Cancer is a name we gave to more than a hundred different diseases. They share one symptom — cells that won't stop dividing — and little else. Every cell in your body carries the same directions and divides on a schedule: replace what wears out, repair what tears, then stop. Cancer is that last instruction failing — a cell that keeps dividing past the point it was told to stop, and passes the malfunction on every time it divides again. The mechanism is as old as multicellular life, because it's the same one that builds a body in the first place. There's no version of a body that can heal a paper cut and can't also make this exact mistake. And this is completely by design.

This is also why there's probably no single cure — "cure" doesn't mean much for a category this wide, any more than a single fix would mean anything for "illness." What this column wants to deal in isn’t certainty, but a set of questions. Screening changes the odds of catching something early enough to matter, nothing more. It works only in the specific places where early actually helps. Vigilance without certainty is an odd instrument to be handed, and most of us are never taught how to hold it: how to be careful without pre-grieving a diagnosis you don't have, how to walk into a screening room informed rather than braced. That's the actual subject here — what it takes to be prepared for something you can't fully know.

Here is a thing almost nobody says to you plainly: the age you memorized for your first mammogram is already wrong. It moved from fifty to forty in 2024, and most of us are still carrying the old number around with no memo telling us to update it. Colorectal screening moved too, from fifty to forty-five, back in 2021 — and most newly eligible people still don’t know it. Lung cancer has a test too — an annual scan, but only if you have a real smoking history — and it kills more women than any other cancer.

Cervical screening has had the busiest few years of any test here: a self-collected swab instead of a speculum, cleared for clinics in 2024 and for home use in 2025, now a preferred option under federal guidance. One catch — full insurance coverage doesn't arrive until 2027.

Ovarian cancer has no recommended screening test at all — because the available tests caused more harm than good, flagging cancer in women who didn’t have it often enough to send many of them into unnecessary surgery. The absence of a test here isn’t the system failing you. It’s the system telling you the truth about what it can’t yet do. Endometrial cancer has the same gap — no routine test — but a simpler answer: any bleeding after menopause gets a prompt call, not a wait-and-see.

Skin is the odd one out: no routine screening is recommended for the average adult, even though most dermatologists will still tell you to get a new mole checked. Both things are true. The data hasn’t caught up to the intuition yet. Mouth and throat work the same way, with a wrinkle. The exam your dentist gives your mouth and throat isn't what the "insufficient evidence" verdict is even about — it's specifically about primary care doctors, and dentists keep checking anyway as routine practice. Throat cancer has no screening test either, and its biggest driver has shifted: oropharyngeal cancer is increasingly an HPV story, which makes the vaccine, not a screening visit, the actual prevention tool. Thyroid cancer flips it again: the guidance here is an active recommendation against screening, because ultrasound finds so many slow-growing cases that the biopsies and surgeries that follow do more harm than the cancer would have — and it’s nearly three times more common in women than men.

All of this — the ages, the intervals, the absences — describes the average-risk woman, which is a real category and also not necessarily yours. A parent diagnosed young, a genetic marker in the family, a prior finding of your own moves every number on this page. So does something the guidance is only recently starting to say out loud: Black women are diagnosed with breast cancer at roughly the same rate as white women — slightly lower, in fact — and die of it at a rate 40 percent higher, a gap that roughly doubles for women diagnosed before 50. It’s driven partly by more aggressive disease and partly by everything that happens, or doesn’t, after a mammogram — which is the case, still being built, for starting the conversation earlier rather than waiting for the standard number to arrive.

The same gap between what feels reassuring and what’s actually been proven shows up somewhere else, too — in a doctor’s office brochure, a wellness clinic’s intake form, promising a single blood draw or a whole-body MRI that looks for dozens of cancers at once instead of one at a time. The appeal is obvious. The evidence isn’t there yet: Galleri, the best-known blood test, still isn’t FDA-approved, and its largest trial missed its main goal of catching more late-stage cancers early. Whole-body MRI, tested in over 9,000 healthy people, found cancer in under 2 percent of them — with biopsy rates swinging from 2 to 22 percent across clinics and no shared rulebook for what a finding should trigger next. These tests are simply ahead of the science that would tell you what to do with what they find. The cheaper, better-tested version of the same idea is one you already have: know your own body well enough to notice fatigue that doesn’t lift, a new lump, weight loss you didn’t work for, bleeding that shouldn’t be there. It won’t catch everything early. Neither, yet, will the thousand-dollar scan.

None of this is a verdict. It’s a starting point, and one worth walking into an appointment already knowing, because a guideline is a floor, not a ceiling, and the difference between the two is exactly the conversation most of us never get to have. Nobody published this plan. Nobody had to. But ask around this week and you’ll hear the whole thing delivered by four different people who’ve never met and would deny, if asked, to ever agreeing on anything.

At school pickup, a woman you’ve spoken to four times in six years mentions, unprompted, that her daughter made varsity and got in early somewhere, then asks, kindly, how yours is doing. You say fine. She says that’s wonderful, in the specific tone that means it is not. At the gym, the woman on the next bike explains her protein target to no one in particular — a gram for every pound, no exceptions, tracked — at a volume clearly intended for you. Near the free weights, someone uses the phrase “strong is the new skinny,” unaware, or unbothered, that it’s been on a t-shirt since 2016.

At dinner, your mother-in-law mentions, apropos of the mashed potatoes, that a friend’s daughter-in-law just made partner. Of course, she isn’t comparing you to anyone, she wants you to know. An aunt asks, with real warmth, whether you ever think about [more] kids, as though fertility were a flight you’d simply failed to check in for. Someone’s brother-in-law, newly into peptides, recommends a mushroom for cognitive clarity and a different mushroom for stress, and, when pressed, concedes the third might just be a mushroom.

On the way out, an old colleague, three years into “building something of her own,” mentions that prevention really should start by thirty-two, books her Botox the way other people book a wedding venue, and posts, later, a sunrise captioned terrified — though she does not look terrified. She looks tan, and extremely well-lit for someone in crisis.

Meanwhile, unmentioned by anyone at the table: the actual parts. A body running its own internal weather system — flashes, fog, a libido that left on a space tour — largely without a script, because nobody at dinner has one for that. Two sets of aging parents, managed in the margins, ideally without either generation finding out you’re frightened. A marriage kept current on no budget of time whatsoever. None of this makes it into the small talk or the social media reel.

We’ve absorbed the whole thing now — the pickup mom, the cyclist, the mother-in-law, the aunt, the brother-in-law, the colleague, et al. We have some notes.

Nobody has ever completed the list — not the protein and the partner track and the vampire facial and the perfect kids, all at once, indefinitely. That was never a coincidence. It’s what a culture does when it needs a woman to keep almost arriving: hands her the same fourteen items and lets her think she’s simply living the norm.

Here’s the part none of them have priced in. She skipped the small talk at pickup. She told her mother-in-law the potatoes were fine and left it there. She didn’t watch the sunrise story twice. She is, right now, on the porch, coffee gone cold, doing the single most subversive thing available to her this week: absolutely nothing, on purpose, no update for anyone. Her response to et al: “I will make a mental note to be deeply concerned about that later.”

The et al, for the record, is still talking. It just doesn’t know yet that it’s talking to itself.

Let’s talk about foods that permit dignity. Steak, certainly. A neat arrangement of knife and fork, small bites, occasional wine. Sushi, if you know what you’re doing. Even soup, sometimes. A noodle, alas, has other plans.

It dangles. It escapes. It flicks sauce onto the white shirt you should have known better than to wear. One end can be halfway down your throat while the other remains firmly committed to the bowl. There comes a moment in almost every noodle meal when you must choose between elegance and eating.

Choose the noodle. Please. We have been doing so for a very long time.

Four thousand years of a very good idea

In 2005, archaeologists working at Lajia in northwestern China reported something extraordinary: beneath sediment, inside an overturned earthenware bowl, were noodles. Actual strands, preserved for roughly 4,000 years.

They were thin, long, yellow and—here is the surprise—made from millet, not wheat or rice. Researchers concluded that people in Late Neolithic China were already grinding grain, making dough and repeatedly stretching it into long strands before boiling it.

Which gives China the oldest physical noodles yet discovered.

It does not quite answer the question everybody immediately wants answered: Who invented the noodle?

China? Italy? The Arab world?

Food historians have spent considerable energy on this. The noodle, having survived four millennia perfectly well without intellectual property counsel, has become a small geopolitical problem. The famous solution offered to generations of schoolchildren was Marco Polo. He went to China, encountered noodles, returned to Venice in 1295 and gave Italy pasta. It is an excellent story. It is also not true.

Pasta was already known in Italy. A Genoese document from the 1270s mentions macaronis, while the Arab geographer al-Idrisi had described pasta production in Sicily more than a century before that. The passage in Marco Polo’s own travels that supposedly proves the Chinese connection describes a starchy substance from a tree—probably sago—which he compared with food he already knew at home.

Marco Polo did not bring spaghetti to Italy. We should perhaps forgive him. He never said he did.

The more interesting answer is also the messier one: like most good things, noodles do not require a single inventor. Give human beings grain, water, hunger and enough time, and apparently someone eventually looks at the dough and thinks: What if we made this extremely long?

The noodle is a map

What happens next depends largely on where you are standing.

Where wheat grew, wheat became noodles. Where rice dominated, rice became noodles. Buckwheat became soba. Mung bean became translucent glass noodles. Sweet potato starch became the extraordinarily chewy Korean dangmyeon. Durum wheat became the great dried pastas of Italy.

And then people moved. The noodles moved with them.

Ramen is now so completely associated with Japan that it can be easy to forget its Chinese roots. Chinese-style wheat noodles were being served in Tokyo by the early twentieth century; Japan took the idea and proceeded to regionalize it with magnificent enthusiasm. Kansui—the alkaline solution that gives ramen its characteristic spring—helped distinguish the noodle from the udon and soba already embedded in Japanese cooking.

Migration can produce entirely new noodles, too. After the Korean War, refugees from northern Korea settling around Busan could not easily obtain the buckwheat used in northern cold noodles. Wheat arriving through American food aid was available. Wheat, mixed with potato or sweet-potato starch, became milmyeon—a Busan noodle born from displacement, scarcity and substitution.

A bowl of noodles can therefore tell you what grew somewhere. It can also tell you who arrived, who left, what was scarce, what was traded, and what people cooked when the ingredient they remembered from home was no longer available.

That is a whole lot of memory for flour and water.

And then you have to eat the thing

This is where noodles become particularly revealing.

Because humanity may broadly agree that long strands of starch are desirable. We absolutely do not agree on what a civilized person should do with them.

Consider the slurp.

If you grew up being told that audible eating signaled the imminent collapse of society, Japan presents a delightful adjustment. Slurping ramen, soba and udon is perfectly acceptable. For a culture with such elaborate codes around public behavior, the sanctioned racket of a noodle shop feels like a magnificent little exception.

And there is something wonderfully liberating about a food culture that acknowledges the mechanical reality of the object. The noodle is hot and it’s slippery. You have chopsticks. Proceed accordingly.

Travel a little farther and the rules rearrange themselves again.

At a Vietnamese street-food stall, the noodle bowl arrives not so much finished as ready for negotiation. Herbs and leafy greens accompany noodle dishes; chile, lime, fish sauce, garlic vinegar and other condiments sit within reach. You adjust. Taste. Adjust again. The bowl becomes yours.

In Korea, meanwhile, there are scissors.

Actual scissors.

They appear at the table for barbecue, kimchi and cold noodles, and nobody regards this as evidence that the diner has failed some fundamental noodle examination.

Although even Korea refuses to make the matter simple. Some specialists insist certain chewy noodles taste better uncut: a Korea.net account of Busan milmyeon notes that chefs recommend leaving the strands whole because cutting them can compromise their texture.

So even the rule about there being no rule has an exception.

Italy, meanwhile, hands you a fork. There is a beautiful confidence in this. Here is your spaghetti. Here is one small metal implement. We have been doing this for some time. You’ll work it out.

Same problem. Entirely different civilizations of the mouth.

The long and short of it

Sometimes the length itself means something.

Chinese longevity noodles—changshou mian—turn the unbroken strand into a wish for long life. Their length is the symbolism. Cutting them rather defeats the message.

Japan has its own version of the metaphor. On New Year’s Eve, toshikoshi soba—literally “year-crossing soba”—marks the passage into the new year. The long, thin noodles have come to represent longevity and good fortune.

Italy would like a word

At some point we must address the question capable of ruining an otherwise agreeable dinner: Is pasta a noodle?

There are several possible answers.

The taxonomic answer is: close enough that a neutral observer can see why the question arises. Special mention here to a former colleague who once looked an Italian straight in the eye and informed him that his carefully constructed pasta was, after all, just another noodle. We shall not go into what followed. It was not a productive afternoon at work.

But the culinary answer to this question is: that is really not the point.

Which brings us to perhaps the most important thing about learning noodles properly: the chew. Texture matters enormously. Italian cooking gave the world al dente: literally, “to the tooth.” The pasta should offer resistance. Taiwan has something even better: Q.

Q describes a particular pleasurable springiness or chew—the bounce of a fish ball, the elasticity of a noodle, the resistance of a tapioca pearl. QQ intensifies it. Michelin describes it as something like an Asian cousin of al dente: soft, but emphatically not mushy; yielding, then bouncing back. Once you notice texture, the noodle aisle changes completely. A glass noodle is supposed to be slippery. A good ramen noodle should push back. Udon has heft. Soba has tenderness and grain. Dangmyeon has that glossy, elastic chew that makes japchae what it is. Rice vermicelli is light enough to disappear among herbs. Biang biang noodles arrive like belts.

And this is why replacing one noodle with another because they are both “Asian noodles” is rather like substituting mozzarella for Parmesan because both are cheese. Dinner will occur. That is about all we can promise.

But when in doubt, here's the compendium we promised — quick enough to consult mid-aisle, right as the unease sets in.

There is no best noodle in this table. That would be a ridiculous thing to suggest. And perhaps this is why noodles have survived every attempt to make them merely fashionable.

A noodle is not precious. Even when making one requires considerable skill—the slap and stretch of hand-pulled dough, the practiced knife cutting soba, the exact alkaline chemistry of ramen—the final instruction is always the same.

Eat it. Before it swells in the broth. Before the sauce congeals. Before the perfect chew disappears.

Someone may have spent hours making the stock. Someone else spent years learning the dough. Behind both may sit a recipe, a migration, a crop, a grandmother, a war, a trade route, an entire argument about whether the strands should be cut.

And then you sit down. Slurp. Twirl. Snip. Twelve minutes later, the bowl is empty.

That is the peculiar art of food: enormous human effort devoted to making something whose highest purpose is to disappear.

And yet the good ones don’t.

You remember the ramen shop at an inconsequential strip mall. The phở eaten on a plastic stool at a dusty Hanoi intersection. The bowl that your neighbor brought in when you were ill. The terrible hakka noodle you had at the generic “Asian street food stall” at a city you’d just moved to. The noodles your mother cooked without ever measuring anything, which you now cannot reproduce despite possessing both the recipe and considerably better equipment.

The noodle disappears. The rest, inconveniently, enchantingly, stays.

Four of us, two milestone birthdays, and a chorus of people — some of whom had actually lived there — certain it was the wrong city, at the wrong time, for women like us. We went anyway.

The trip began as an excuse. Two friends had birthdays with a number on them, the sort that seems to demand a passport, and four of us wanted to be somewhere together for it. Where was still open. I closed it. It had to be Rio, and it had to be Carnival — and the moment I said so out loud, I found I had proposed something closer to a public-order offense.

I could write a book about the audience it drew, and one of these days I might. The warnings came from every direction and in every register: wrong city, wrong week, wrong idea entirely, and — always, eventually — the wrong thing for women. Some of the most discouraging came from the people who knew the place best: born there, raised there, long since moved away, who spoke of Rio the way you might speak of a beautiful and volatile ex — with real love, and a firm recommendation that I keep my distance.

I had the answer ready, and used it often. I grew up in India. I know how to move through a crowded, complicated place.

Which is the distinction the whole chorus kept missing. Every warning had conflated two different instructions into one — careful and away — as though the only safe form a woman could take was a stationary one. Being careful and staying away are not the same instruction. We went. We were careful. Both are true, and being careful never once required staying away.

The first bloco swept us up before we were properly ready for it, which is the correct way to meet one. It was Carmelitas, the parade that opens Rio’s street Carnival on the slopes of Santa Teresa, and it runs on one of the better legends going: a cloistered nun who, the story has it, jumped the convent wall each year to join the party and slipped back before anyone noticed. So everyone marches as nuns — the better to hide the runaway in the crowd — thousands of us, in habits, climbing a cobblestoned hill in the heat behind a truck carrying a drum section and a woman singing something the whole street already knew. There is no standing to one side of a thing like that. Within about four minutes you have stopped wanting to.

The birthday friend is the part I keep returning to. We landed and went more or less straight into a bloco, and she — whose week this was — put the costume on but not much else: no makeup, no accessories, in it without being in it. Something about it felt like it belonged to a version of herself she thought she’d retired; she wasn’t sure; she hung at the edge of the thing, half-dressed for it. That lasted exactly one afternoon. From the next morning she was the most committed nun in Rio: fully turned out, every day, every dress code for every bloco honored to the letter, more devout in her costuming than any of the rest of us. I have thought about that conversion more than once since. She didn’t decide her way into the week. She went anyway, unsure, and the place did the rest — which is, more or less, the whole argument for going.

It raises a question I’ve been turning over since we came home: why do any of us go anywhere? You go to see a thing you’ve never seen. You go to stand inside a way of living that isn’t yours and be, for a week, the one who doesn’t know the rules and doesn’t mind. You go because something in you leans toward a place long before you can explain why. You go for reasons that don’t flatter you in the least — to look, frankly, at beautiful bodies, of which Carnival provides a generous surplus. You go to be near art, or food, or noise, or a grade of joy your own sensibly arranged life will never quite manufacture on its own. Not one of those reasons is improved by staying home to be safe.

Somewhere between the morning and the night, you have to eat, and you should eat the way the city does: at a rodízio, the churrasco arriving in rotation, meat and then seafood and then more of both, until you wave the whole parade off. Then go looking for the plates the tourist doesn’t — the stews, the fried things, the dish someone’s grandmother clearly had opinions about. They are everywhere, if you look where the locals are looking.

And the samba is everywhere too — at the roadside, in the blocos, on the beach, and, most decisively, at the Sambadrome: the temple of the whole thing, and a night unlike anything else you’ll see. Go once, however you can get in. Dress the part. Arrive already knowing you’ll dance.

The best night of the trip nearly didn’t happen. We took an Uber to Rio Scenarium — three stories of live samba and antique clocks and chandeliers, in the middle of Lapa — and then sat in the car at the curb, four of us, not getting out, because the street outside read exactly the way we’d been warned all of Rio would read. What finally moved us was, of all things, the police: actual officers standing on the road, which we took, rightly or not, as a sign that this was a corner the city kept an eye on. We got out. We went in. We had, without exaggeration, the night of our lives. The judgment that got us out of that car was not recklessness and it was not fear — it was the same quick reckoning you’d run at home, made consciously in a place you don’t yet know by heart.

So let me be plain about the word, because I am not selling recklessness and never will. You take every precaution there is. You read the street, you mind your things, you make the hundred small adjustments a capable adult makes anywhere she isn’t yet fluent. And some rooms you simply choose not to walk into — the favela baile everyone else seemed to want, in our case, which we left to the people who actually live there. Safety is not the toll you pay to travel; it’s the thing that lets you eat the whole of it and come home greedy for more.

It was, and I don’t say this lightly, one of the best decisions I have made — not the best trip, the best decision, which is a larger and more useful thing, because a decision is something you can make again. And I have.

Rio does not lower its voice for anyone’s comfort. For one week, four of us didn’t either — and the world that had been so certain we shouldn’t be there turned out, in the end, to simply be somewhere else.

Certain words behave differently depending on who they land on. Said of a man, they are neutral, sometimes admiring — a description of a way of being, no more. Said of a woman, the same words acquire a second meaning, a verdict, a small clinical chill, and begin to function less as observations than as labels — meaning: adjustments are required, or else.

The evidence is easiest to see at altitude. Consider the woman in public life who is decisive in precisely the way the man beside her is decisive — and who is called something sharper for it: calculating where he is analytical, cold where he is disciplined, ambitious where he is simply focused. We do not need her name. We supplied it while reading the sentence, which is the entire point: the pattern is legible enough that a face arrives unbidden, and no equivalent face arrives for the man.

And here is the complication that keeps this from being a tidy grievance. Some women have learned to lead hard on purpose. One executive, asked why she led with her chin in every meeting, said flatly that she had spent a decade being talked over and had simply stopped waiting for space that was never offered. She was not born abrasive. She had had to tune herself — gradually, deliberately — to the one frequency that carried over the clamor of the room, and was then handed the verdict for the tuning as though it were a flaw in her character rather than a solution to the room’s. The words that follow cannot tell the difference between a woman who is truly difficult, a woman who was made difficult in self-defense, and a woman who is only so labeled. That inability is how a fix gets recorded as a defect.

So: four of many artifacts, probed below. Each entry notes the same three things — the meaning the word carries when it is doing what it’s supposed to; the conditions under which the trait is genuinely insufferable, man, woman, or otherwise, because pretending otherwise would be naive; and the method by which the word, examined at close range, tends to fall apart in the hand of the person voicing it.

Too Ambitious

In use: A word that curdles into a warning only when its subject turns out to have more of the quality than the speaker had braced for. Applied to a woman, ambitious is less a description of her than a measurement of the distance between what she wanted and what she had been quietly assigned to want. The gap is the offense. The word only names it.

Genuinely insufferable when: ambition arrives detached from any interest in the work itself — the person who wants the corner office and none of the labor that is supposed to lead there, who treats colleagues as rungs. That exists, and it grates, in anyone. But note how rarely ambitious is the word spent on him when it does.

Fielded: The response is not to deny it, because denial concedes there was something to be ashamed of. It is to accept the word without a flicker and ask, warmly, what the preferred alternative would have been. No one has ever completed the sentence we would have liked you to want a little less out loud, in daylight, and remained standing.

Overly emotional

In use: A word one or two removes from a literal medical diagnosis — hysteria, the wandering womb — and not as many removes as you’d hope. Show feeling and you are emotional; withhold it and you become cold; the strip of sanctioned warmth between the two is narrow enough to fail on an ordinary Tuesday. Which tells you the word is not measuring the reaction at all. It is measuring the observer’s comfort, and filing the result as a fact about her.

Genuinely insufferable when: the feeling is a tactic — the performance of distress deployed to end a conversation or relocate its blame. That, too, exists, in anyone. It is also vanishingly rare compared to the number of times the word is used to describe a reaction that was simply the correct size.

Fielded: The reply is a single question, asked levelly — which part, exactly, was disproportionate: the feeling, or the thing that caused it? The accuser has usually not thought past the feeling, and rarely enjoys being escorted back to the cause.

Not a team player

In use: Dissent, entered into the permanent record as a defect of personality. Original thought, refusal to ratify the room’s consensus, not appreciated. The recategorization is the whole maneuver: a disagreement about the work is refiled as a flaw in the woman, where it can be managed by HR rather than answered on the merits.

Genuinely insufferable when: the person really is a menace to the collective — hoarding credit, withholding information, treating the people beside her as terrain to be crossed. This is real, and it is intolerable in anyone: man, woman, child, or, on a sufficiently bad day, animal. But that person is loud about herself. The woman the phrase usually lands on was loud about the problem, which is a different thing wearing the same word.

Fielded: Ask what, precisely, the team was playing — and whether the difficulty was the person, or the fact that she was right where everyone could see it. A team that functions only for as long as no one disagrees is an arrangement, and an anxious one. Naming it as such tends to conclude the meeting.

Lacks executive presence

In use: The most elegant of the four, because it arrives pre-equipped with a definition and still manages to mean nothing. The authoritative version belongs to the economist Sylvia Ann Hewlett, who resolved executive presence into three parts: gravitas (how one carries authority and holds up under pressure), communication (how one speaks and commands a room), and appearance (how closely one matches the picture of a leader already in the viewer’s head). A genuinely useful taxonomy — right up until you ask how any of the three is measured, at which point all three dissolve into the same answer: by the gut of whoever is doing the looking.

Gravitas is poise, as read by the reader. Communication is articulacy, as judged by an ear tuned to a particular frequency. Appearance is resemblance to a template one is simply used to looking at. The framework does not measure presence; it sorts a single instinct into three bins and returns it wearing the authority of a rubric. Its natural habitat is the withheld promotion and the truncated presentation — one manager cut a woman’s presentation short, wanting the room’s attention for himself, then recorded her lack of presence as the reason she had failed to hold it.

Genuinely insufferable when: Never. But here the concession can begin, because there is a real thing at the bottom of the first pillar — some people genuinely come apart under pressure, cannot hold a decision, cannot make themselves understood in a room that needs them to. That exists, in anyone, and it is a real limit.

But watch what the term does with it: it binds that one measurable deficit to “sounds like us” and “looks the part,” scores the bundle as a single impression, and lets the assessor lean on whichever of the three justifies the conclusion he reached before she opened her mouth. The honest kernel is the alibi for the other two.

Fielded: Disassemble the finding. Ask which of the three — gravitas, communication, or appearance — was found wanting, and by what measure. Press gravitas and it becomes did she seem sure of herself, to me. Press communication and it becomes did she say it the way I would have. Press appearance and the honest answer, said aloud, is usually a description of a man. The aim is not to win the exchange. It is that a standard which cannot survive the question by whose measure? was never a standard. It was a preference with a corner office.

Read the four together and the shape is the same. None is a measurement of the woman. Each is a confession the room made without meaning to. So when it arrives, turn it around, without heat. And ask what it’s measuring. You owe it nothing.

Say it first, because it doesn’t sound the way it looks to an English eye: roughly hir-eyeth — the first syllable short, not hire — the last landing like the end of breath. Ask a dictionary and it will tell you hiraeth means longing and, well, period; but longing is the word English reaches for when it hasn’t got the right one, an all-purpose ache and want applied to a lover, a holiday, a sandwich, with equal ease. Hiraeth is a specialist’s tool. It names one exact thing: that you love a place, that you need to be in it as something close to visceral, and — the part English keeps mislaying — that you know, even as you feel all of it, you cannot go back. The three don’t arrive in sequence. They arrive fused, which is why one word can carry what an English sentence has to lay out end to end. Homesickness is too curable. Nostalgia is too sweet. Hiraeth writes the impossibility into the definition: the door is shut, the longing is for the room on the other side of it, and part of what you feel is the shut door.

But here is the fun in hiraeth — it’s that the place you feel it for need not have existed. You can feel hiraeth for a home that was never yours, a time that ended before you arrived, a version of somewhere, someplace, someone that your memory has quietly improved past the point of fact — it will attach to a country you left, or to one you only ever heard described, warmly and at length, by someone who left it before you were born. And it refuses to stay put on a map. The object can be a place, yes, but as easily a time, or one particular hour of one particular day; a person no longer reachable, whether by distance or death or the plain fact that neither of you is who you were; or a thing so small it embarrasses you to grieve it. The word does not rank them. It only insists the ache is the same ache, whatever it has fastened onto.

The word has been having a moment lately, and not an entirely good one — it turns up on the tote bag and in the caption now, filed under beautiful untranslatable words, stripped down to a soft, sellable wistfulness, a cozy sort of missing you might feel for a cottage weekend. That is a small theft, because the real word is not cozy: it is a word about not being able to go home, and the raw need, pain, want, and love that the feeling generates. Blurring that into a mood is reductionist and that, in the end, defeats the whole reason the word exists.

It exists because a feeling you can name is one you can hold a little away from yourself and up to the light — a good deal more bearable than carrying it shapeless. Everyone has a hiraeth. The Welsh only did the rest of us the courtesy of giving it a shape. Now you have the word for it.

There is a thing that happens in the car. A song arrives in my head — half of it, the good half, the part everyone remembers — and I begin to sing it. And before I reach the end of the line, without a word, my husband finds it on the screen. The real one. Cues it up at volume, and the actual singer arrives to take the song off my hands.

He is not unkind about it. There’s no actually, it goes like this, no lesson, no sigh. He simply produces the correct version and lets it play, the way you’d hand someone the right tool without comment. The song I was reaching for arrives, sung properly, by a professional, and my own goes quiet under a louder, more perfect one.

My husband can sing, is the background to this. Not shower-sing — actually carry a tune to where it was going and set it down intact. I cannot. I am, and I have made my peace with the ranking, the best in a row of bad singers — which is a category of compliment that dissolves the second you look straight at it. For twenty years the right voice has been quietly replacing mine in moving vehicles, and for most of them I received it as help.

It took an embarrassing while to notice that the help might not be for me.

Because here is what a wrong note does to a person who sings well: it hurts them, the way a picture hung crooked hurts you until you straighten it. He isn’t teaching me — I’ve proven, across two decades, unteachable in this exact way — he is relieving something. The gesture is aimed at me, but the relief it brings is his. I say this without grievance; we’re long past the years when it would’ve been a fight, we are still very much in love, and I can watch him do it now with faintly scientific amusement rather than a slammed door. There’s a specific small comedy in being edged out of your own song by a machine — a whole living woman, replaced mid-verse by the definitive recording.

And I had a whole essay ready to go from there. You can see its shape. The one about how we should all be allowed to make the joyful wrong noise, do the thing without being good at the thing —

I can’t write that essay, though, and the reason is the other thing I do — the thing I love too much to leave alone, that I direct on nights and weekends for audiences who have paid to be there.

I direct dance — not professionally, but with something more compromising than a paycheck at stake. It’s the thing I chose, and I made myself its bouncer. The room where I love it most is also the room where I stand at the door and decide who is good enough to be seen doing it. I hold auditions. And at an audition I don’t correct anybody either; I do exactly what the screen does. I replace them. I watch someone dance with their whole heart, not quite rightly, and I think, with total clarity, no — and I find the one who does it properly and that dancer goes on stage instead. He substitutes the right voice for mine in a car. I substitute the right body for the eager one in a theatre. Same gesture. Mine has a lighting budget.

So I don’t get to write the pretty essay. The woman who wants to be let into singing on no qualification is the same woman who guards the door to the dance she loves so much.

I have tried the exits, and none of them holds. Singing is joy, dancing is craft — but I dance craftlessly in the kitchen for no one, and I’d be insulted if you filmed it and sold tickets. The car is private, the stage is public — but I have silently judged a stranger for singing flat two tables over. It’s only a side thing — except the side things are the ones you chose, which makes the strictness worse; nobody made me guard this door, I volunteered. Every distinction is true, and every one leaks. I am tender about the filter when it’s aimed at me and ruthless with it when I hold it, and I have not found the version of myself who reconciles those two women.

What I keep returning to isn’t the hypocrisy. I can live with a hypocrisy; most working lives are built on one. It’s the quieter thing under it: I want, more than is quite reasonable, to be the one who sings well. Not to be let to sing — I already am, the car is a democracy, the phone can’t actually stop me — but to be good at it. To open my mouth and have the right note come out, no professional required, no recording necessary. I hold that standard so fiercely, in the room I love most, precisely because I know what it’s worth. I have no intention of lowering it — not for the dancers, not for anyone. I would simply like to be excused from it — just me, just the singing — and I would like that very much.

I am not proud. I am, however, still singing.

This is the first of these, so we should start with the history most money columns are too well-mannered to mention.

For most of the time money has existed, women had none — not by oversight but by design. She married, and what had been hers became his; if she earned, her wages were his; the law across much of the world said so without embarrassment. And it ran deeper than money. In a great many places, across a great many centuries, a woman was not a person forbidden from owning property. She was the property — chattel, a vessel for continuing a man’s line, a thing that moved between households with a settlement attached. It is recent enough that the residue is still in the water, and the residue has a modern name.

The name is the flinch — that small internal stammer a woman still feels quoting her worth without shaving it first, asking for the raise, saying out loud what she earns, talking money at a table where the men are doing it untroubled. We’re taught to read the flinch as good manners, but it is inheritance: the long echo of centuries in which money was, quite precisely, not her business, on the grounds that she was nearer to being the goods. You didn’t invent that discomfort. It was issued to you, and most women carry a share of it without ever being handed the receipt.

The standard portrait of a stable financial life, then, was never sketched with her in the room. Look up financial stability and you’ll get a version of this: a state in which a person can comfortably meet their costs, service their debts, and take a shock without living in constant money stress. Dependable income, controlled spending, a safety net. A perfectly sound definition that skips the one question deciding everything for a woman, and skips it with a straight face. It asks whether the income is dependable. It never asks whose it is.

Because by that definition, a woman with none of her own money is stable, so long as somebody else’s arrives on schedule. She passes every test, owns not one line of it, and hasn’t the faintest idea what becomes of her the day the arriving stops. The measure checks whether the bills get paid, not whether the woman paying them is the one who decides — the same assumption the whole history ran on, now hiding inside a dictionary.

So let’s settle what it means for money to be yours, since everything turns on it. It has nothing to do with whose hands earned it. Money is yours when you hold the right to decide what happens to it — spend, save, invest, give away, or refuse to part with it — without anyone’s sign-off. A woman in a real partnership, with genuine say over what the household does with its money, owns it by this measure whatever payslip it arrived on. A woman whose every comfort is provided but who must ask, itemize, and justify does not, however handsome the provision. Ownership is who decides, not who earned and not who benefits. Hold onto that; the whole ladder stands on it.

So we need our own measure — a truer one, not a gentler one. It starts with what stability is not: it is not being rich. That mix-up has kept a lot of capable women awake over a target they were never chasing. Stability isn't a number trailing zeros; it's the point where ordinary life stops feeling financially fragile — where a surprise bill is an annoyance rather than a catastrophe, and a bad month is weathered rather than survived. Rich is a place almost nobody arrives. Un-fragile is a position most women can build on purpose, in stages. They look like this.

Level One — the month holds. Income beats outgoings. Rent or mortgage genuinely affordable, not white-knuckled on the first. Bills paid on time, groceries covered, transport you can count on, no payday loans, no credit-card balance rolling forward — and something left over, however slim. Everything else is built on it.

Level Two — the cushion. Where resilience starts. One month’s expenses saved, then three, eventually six — more if your income is self-made and moody. Health cover; disability cover if your income depends on you turning up to earn it; life cover if someone depends on yours. High-interest debt brought to heel. Life will still throw a punch, or five. You simply stop being knocked flat by each one.

Level Three — it runs without you. The machinery stops needing daily supervision. Saving happens on its own; so does investing. The emergency fund sits untouched, because it’s for emergencies and this — the sale, the sofa — is not one. Credit healthy, nothing missed, debt shrinking rather than sidling upward, spending aligned with what you value rather than what you felt at nine in the evening. Perfection was never the aim. Predictability is.

Level Four — it grows. Wealth accumulates quietly while your attention is elsewhere. The vehicles vary, and this column won’t prescribe them from a single list — retirement accounts, index funds, the employer match you’d be unwell to leave on the table, property where it fits, equity. Each deserves its own hour, and will get one in these pages — consider this the table of contents, not the meal. And underneath every rung is the one asset no list of vehicles includes: your own earning power. The skill that lets you charge more is among the best investments anyone makes, and the only one nobody can quietly transfer out from under you.

Level Five — the reason for all of it. Here money stops being about things and turns into choices. Can you leave the job that costs more than it pays? Take six months out? Care for a parent, help a child without capsizing yourself, start the thing you've circled for years? Say no — to the client, the situation, the person — and mean it, and afford it? That is the real wealth: not a heap, but a corridor of doors that open when you lean on them. Its place at the top is no accident, because to leave, to refuse, to provide, to go are exactly the freedoms the history withheld. Optionality is no flourish. It is the old inheritance, taken back.

Which brings me to the part the sensible articles leave out. None of this is a vow of austerity. Money that is yours isn’t for hoarding in a joyless pile while you admire your own restraint — it’s for spending, some of it, magnificently, on things you wanted and earned and chose. There’s a specific pleasure in buying something with money that is unmistakably your own: the coat or the meal or the flight that is yours twice over, once because you bought it and once because you made what bought it. The freedom worth building isn’t only the freedom to walk away from things; it’s the freedom to walk toward them, with no knot in the stomach and no case to plead. That is not the opposite of building wealth. It is what the wealth was being built for.

So: not rich. Un-fragile, and rising. Some women who look precarious on paper turn out steadier than anyone at the table, and some who photograph beautifully are quietly sliding. You’ll finally know which you are, because you’ll be measuring the right thing — not whether the bills got paid, but whether the woman paying them is the one who decides.

For one month this summer the football came to us — forty-eight countries, three host nations, sixteen cities strung between Vancouver and Mexico City — and then, on a Sunday in New Jersey, it was over and Spain had won.

What takes hold is the turning. A whole cross-grained, disagreeable planet wheeling around for one month to face the same direction at the same time, and you fall in without meaning to. You do not need to tell a back four from a bake sale (we can’t), and still you end up with a team you’ve decided, on no evidence at all, is yours — chosen for the color of the shirt, the face of the manager, a grandmother from the right country, or sheer contrarian loyalty to the smallest nation in the draw. We adopted Cabo Verde — half a million people, at their first World Cup — and defended them warmly to people who had not asked. That we had no business doing this was the whole pleasure of it.

And the delights kept coming. The flag that went up on a balcony two doors down overnight. The ninety-four-year-old down the road who had to be brought up to speed, every day, on who was playing in the final of this “new sport” everyone is so fond of. Half the world sitting down in rows to heave on imaginary oars for a chant invented this spring that looks a thousand years old. Grown men weeping with their entire faces (the blasphemy!), on live television, about a game, and no one telling them to compose themselves.

And then the ending we intend to keep. The winning country matters less than the man who settled it. Ferran Torres, who did not start, came off the bench and scored the one goal that mattered, then said he had struck it “with the power of all the Spanish people.” A substitute. The credit handed straight back to everyone watching. Say that out loud and try to stay unmoved.

That is the least cynical thing left on the calendar. For one month the whole planet agrees to care — loudly, in tandem, about the same small thing — and you get to stand inside that agreement, knowing nothing, earning nothing, wholly part of it for as long as it lasts.

We are smitten. Pull up a stool.

Red is demanding. It asks something of you, nine times out of ten. Wear it and you have agreed, whether you meant to or not, to be seen — and a great many of us have a complicated relationship with being seen, which is why a great many of us do not wear red at all.

The difficulty isn’t flat, though. It climbs. Red shoes, most of us can manage; they’re far from the face, deniable, halfway to a joke. A red bag, fine. The red coat asks a little more, and gets talked out of more often. The red dress asks more still. By the time the red reaches the mouth it is asking the most it will ever ask.

Consider the two women the culture keeps for this. Marilyn, whose whole face was organized around the red mouth, the lipstick doing the talking her voice was too soft to. And Taylor: the same deep red, decades apart, and you cannot quite say what it’s doing on either of them — armor or announcement, the loud thing a managed woman is allowed or the quiet point she’s making without raising her voice. That you can’t tell is the whole interest of it. Two women who could not be less alike, reaching for the identical tube.

So the mouth is where it all concentrates. It is where even the women who wear red everywhere else finally hesitate, and where the women who wear it nowhere feel the whole business start — at the lips, and working backward.

Somewhere in most drawers is the evidence. Several reds, not one — a scarlet bought in a decisive mood, a brick that felt safer, a blue-red someone swore was universal, a berry for later. Ten, if we’re counting, and we are. Ten shades acquired over years, each one still capped, still pristine, still perfect.

The shade was never the obstacle. A woman who owns ten reds and wears none of them does not have a shade problem. She has something else — and it is worth being careful about what we call it.

Here is what makes the lip the far end of the scale, and it isn’t vanity. The mouth is the part of you that is most you — where the words come out, where you eat, where you are kissed, the first thing another face reads. Color on the shoe decorates. Color on the mouth declares. Red there is something you announce, and the announcement is always the same: I am here, I meant to be looked at, I chose this.

That is a large sentence to say before nine in the morning. Some of us are not sure we have cleared ourselves to say it. And some of us simply decide not to. That is a different choice altogether.

Because here is what the drawer is not. It is not a waiting room. It is not ten failures to wear a lipstick, ten deferrals, ten someday-when-I’m-braver. Look at what a woman actually did: she was drawn to the red, and she bought it, and then she was drawn to it again, and bought it again, ten times over, across years. The wanting is real, and repeated, and entirely hers. She just keeps it where no one can see it — renewed every time she brings another one home, and answerable to nobody.

There is an argument that this is the more intimate red. The worn red is a statement made outward, to a room. The drawer of ten is a conversation a woman is having with herself, ongoing and unfinished and private, and no one’s business but her own. It would be a strange kind of progress to talk her out of it.

So we won’t. If you own ten and wear none, this is not the paragraph that fixes you, because you are not broken. And if you own ten and want, one of these mornings, to put one on — that is yours too, and there is a map for it below, sorted not by your undertone but by the size of the thing you feel like doing.

But first, the question the drawer actually asks. When you buy the tenth one, what is it you think you are buying?

And then, because someone will always ask for the practical one.

We’d hoped to get through this without it. But utility is utility, and the undertone chart — unchanged since Carole Jackson put it in every drugstore and every magazine your mother read in 1980 — remains stubbornly, annoyingly correct. So here it is, played straight. Match your undertone; ignore your nerve. That part, as established, is on you.

You have the reds. You’ve always, always had the reds. Worn or not, they were always yours.

So I stopped waiting.

The Yellow Cycle is what happens when a woman who has always known she could write finally decides that knowing is no longer enough. It’s for the women I see everywhere — at the ramen place, at the school gate, in the meeting room, in the mirror — who are more interesting than the culture currently has the attention span to notice. Who have taste, and opinions, and a sense of humor that has only sharpened with use. Who would like, for once, to be spoken to as the adults that life has made them.

We’ll talk about art and bodies and money and work and the people we love. We’ll talk about the things nobody puts on a greeting card. And we will laugh — properly, uproariously, the kind of laugh that startles the table next to you. And apologizes to none.

Hallelujah. We have been located.

For years the algorithm looked straight through us — past us, around us, occasionally at the woman ten years younger standing directly behind us — and showed us nothing. We did not exist as a consumer. We were a gap in the data, a demographic that had stepped behind a curtain and pulled it shut.

Then somebody in a meeting said “Total Addressable Market” and “spending power,” and the curtain flew back.

You’ve seen the ads. A woman roughly our age stands in a kitchen with no visible function — no school bags, no unanswered post, no single useful crumb — laughing alone at a salad. Not a smile. A full, head-thrown-back laugh, directed at lettuce. A voice that has been to a great deal of therapy says the word permission. You have permission now. To rest. To bloom. To “put yourself first,” a phrase that always arrives attached to something costing $58.

We did not ask for permission. We have not, broadly speaking, asked for permission since 2003.

The serum is the purest specimen. They’ve wised up — the twenty-six-year-old is gone. Now they send a woman who looks exactly our age, not a line on her face, to explain how the lines have disappeared. The product did that. Not genetics, not lighting, not a filter sophisticated enough to smooth a walnut. The serum. We are to find this relatable, and then spend the money. The audacity is, genuinely, breathtaking. We almost respect it.

And then there’s the candle range. There is a candle for every internal weather system a woman might experience — one called Unbothered, one called Permission, one that is simply the smell of a woman who has stopped apologizing, retailing at $42 and burning for nineteen hours, which is roughly the apology’s actual half-life. Someone named these. Someone approved the copy. Everyone kept a straight face. We find this heroic.

Here is what we will say in the brands’ defense, because we are fair: they noticed us. After years of pretending that we evaporated at the school gate, someone looked up from the spreadsheet and said “untapped”, and now there are catalogs. Small mercies. We will take the catalogs. We will also light the candles. We will not, however, be taking instructions from either.

The reason most of us don’t own art isn’t money. It’s a quiet conviction that collecting is something other people do — people with a spare wall the size of a tennis court, an advisor, a vocabulary. So we buy the framed print from the homeware shop, the one currently hanging in four hundred thousand other sitting rooms, and we tell ourselves we’ll get the real thing later, when we’re the kind of person who does that.

Here’s the thing. You already are.

Know what you’re actually buying

When you start looking, three categories of work will come at you, and the differences between them matter more than the prices.

A reproduction — also called an open edition — is a photograph of an artwork, printed as many times as the market will take. The poster, the homeware print. Nothing wrong with it, but it’s décor, and it will never be anything else.

A limited edition print is a different animal: a work the artist made as a print — a screenprint, etching, lithograph, linocut — produced in a fixed number, usually signed and numbered in pencil in the margin (you’ll see something like 12/50, meaning the twelfth of fifty). The number is the point. When they’re gone, they’re gone, and you own one of a finite set of objects the artist actually touched.

An original is exactly that — the singular thing. A painting, a drawing, a work that exists once and only once. More within reach than you’d think, especially from artists early in their careers.

If you remember one distinction, make it reproduction vs. everything else. It’s the line between owning a copy and owning a piece.

Two things nobody tells you

The frame is half the result — budget for it, sometimes it costs more than the art. Second, buy what you can’t stop looking at. A $50 acrylic from a thrift store that stops everyone who walks into your living room for twenty years has outperformed every piece bought with one eye on resale. You are not a hedge fund.

Start with one

Not a wall. One piece of art. The mistake is treating collecting as a project to be completed, when it’s a habit to be acquired. Buy a single piece you love, live with it for a season, notice what it taught you about what you respond to. The second piece will be smarter than the first. By the fifth you’ll have something nobody can buy ready-made: a wall that could only be yours.

That Dalí our editor stood in front of for twenty-five minutes? You’re likely not going to own it. But the impulse — I want to keep looking at that — is the only qualification collecting has ever required. The rest is just knowing where to stand.

Here is a thing almost nobody says to you plainly, so we will: the years before your last period are often stranger than the event itself, and they can begin the better part of a decade ahead of schedule.

Menopause is a single point — the day twelve months after your final period, which you can only ever identify in hindsight. Everything leading up to it is perimenopause, the long runway, and it does not keep a timetable. The average woman reaches menopause around fifty-one or fifty-two, but the symptoms can start in your mid-forties, and the transition runs three or four years for some and closer to a decade for others. Two women the same age can be living in completely different weather.

And it can start earlier than anyone tells you — forty is not too young to be in it. The list is also longer than the one you were handed. Hot flashes and night sweats, yes — but also periods that turn up early, late, biblical, or as an ambush; sleep that frays at three in the morning; a memory that drops the word it wanted mid-sentence; moods with their own forecast; a libido that quietly left the building; and the genitourinary business no one says out loud — dryness, urgency, discomfort. If you’ve had half of these and been told you’re fine, you weren’t imagining it.

Two more things the list rarely includes: bones and muscle. Estrogen plays a significant role in maintaining both, and as it declines, bone density can drop and muscle mass quietly follows. This is not inevitable — it is addressable, and earlier is better than later. The evidence is consistent: resistance training twice a week, progressively increased, protects both. Protein intake matters more than most women realize at this stage — current guidance sits around 1.2 to 1.6 grams per kilogram of body weight per day. Calcium and Vitamin D are the unglamorous workhorses of bone health and worth discussing with your clinician if you haven’t already. It doesn’t require a complete overhaul. It requires a start.

One genuinely useful thing to carry into the appointment: if you’re over forty-five with these symptoms, you usually do not need a blood test to confirm any of it. Hormone levels swing so wildly from day to day that a single reading can’t pin them down, and a normal result doesn’t mean nothing’s happening. The diagnosis comes from your history and your symptoms — so a doctor who declines to test you isn’t brushing you off; the test is the unreliable thing, not you. (Before forty is a different story, where testing genuinely matters, because early menopause is worth catching.)

And there’s been a real shift worth knowing about. For two decades, every form of menopausal hormone therapy carried the FDA’s sternest “black box” warning — a hangover from one alarming 2002 study later understood to have largely followed women well past menopause, on a formulation since fallen out of use. In early 2026 the FDA removed that warning’s language on heart disease, breast cancer, and dementia, keeping only a narrower caution about the uterine lining for one kind of estrogen. The current understanding, in plain terms: for many healthy women who begin near the transition — under sixty, within ten years of it — hormone therapy is the most effective treatment going for hot flashes and the genitourinary symptoms, and the risks are low. Begin much later, or much further out, and the arithmetic changes.

The other question nobody answers: how long do you stay on it? The old instruction was “lowest dose, shortest time” — guidance since revised as inadequate and in some cases harmful. The current answer from The Menopause Society is that there is no fixed limit. Duration is a conversation between you and your clinician, based on your symptoms, your history, and your quality of life. Some women take it for two years. Some take it for twenty. Neither is automatically wrong.

This is not a flare fired into the sky. The same clinicians who welcomed the change have cautioned against swinging from one tidy story (“it’s dangerous”) to the opposite (“everyone should be on it”). If you’ve had breast cancer, or it sits close in the family, the picture is genuinely yours alone and worth an unhurried conversation. The compounded “bioidentical” pellets and troches sold at some wellness clinics fall outside both the evidence and the regulators — we’d give them a miss. And low-dose vaginal estrogen, the quiet workhorse for dryness and bladder symptoms, is about as low-risk as anything in the cabinet.

None of this is a prescription; it’s a vocabulary. The reason to say it out loud is that you walk into the room able to ask sharper questions — ideally of a clinician who actually trained in menopause, and there are more of them every year. The conversation is finally growing to the size of the thing. About time.
